We offer a structured Sliding Scale Program for patients who qualify based on financial need. Eligibility is determined through a formal application, income verification, and review process. All available insurance and third-party coverage must be utilized prior to consideration for program eligibility.

Eligibility for the Sliding Scale Program is based on financial need and participation criteria.

For applicants who are not medically unable to work, verification of consistent employment or active participation in work-related activities (generally full-time employment) is required as part of the qualification process.

Patients who are unable to work due to a documented medical condition may qualify for alternative consideration with appropriate clinical documentation.

Eligibility criteria apply, and not all applicants will qualify.
