1. Pay a $20 fee to participate in our virtual addiction treatment program. This monthly fee is not required for onsite or in-clinic treatment.
2. Submit your intake documents.

We will contact you quickly to schedule the soonest available appointment. (during normal business hours)

Please note: Co-payment fees will be determined prior to first appointment and are the responsibility of the patient.
