Submit Your Intake Documents for Telemedicine Addiction Treatment (Using Suboxone)
To schedule a Suboxone clinic or telemedicine appointment, please click here to fill in your intake documents.
Your First In-Clinic Suboxone Appointment:
- Bring your driver's license or state-issued photo ID along with your health insurance cards (if applicable)
- An email and SMS with directions will be sent to you after we receive your intake documents.
Your First Telemedicine Suboxone Appointment
- Be in a quiet, well-lit place and connected to high-speed wifi.
- Open the Recovery Care Spruce Health mobile app.
- Send us a brief message to check in for your appointment.
- You’ll then visit privately with:
- A licensed counselor
- A Suboxone doctor
- Your Suboxone medications will be sent to your local pharmacy and ready to pick up (same day).
- Or, you may have your medications shipped to your door for a nominal delivery fee. - Your follow-up appointment will be scheduled.
- A drug screening kit will be mailed to you within a few days of your first appointment. Please have your drug screening kit with you at all future appointments.

Why Do I need to Submit Intake Documents?
Your intake documents are a requirement for receiving Suboxone telemedicine treatment. They provide our Suboxone doctors with a clear picture of your drug use, drug usage history, current and past medications, family background, family dynamics, and much more. This information is critical to creating a successful recovery plan and determining the most appropriate first steps for transitioning to Suboxone.
If applicable, Recovery Care will establish a relationship with your primary care provider (or other providers) to ensure the best recovery plan possible.
