1. Pay a $20 fee to participate in our monthly telemedicine addiction treatment program.
2. Submit your intake documents.

We will contact you quickly to schedule your appointment as soon as possible. (during regular business hours)

Please note: Insurance co-payment fees will be provided to you before starting treatment. Blue Cross Blue Shield Network E and Network L insurance plans are not accepted. We accept all Medicaid plans but cannot accept Medicare plans at this time.
